Secure Form Distribution
Each school will receive a private, secure Google Sheets link from their local SO contact.
Links will be sent directly to each school’s main contact during the week of September 9th. New schools will receive their links within 7 business days of joining during the school year.
Ongoing Registration
This Google Sheets link will serve as your registration form for the entire school year—no additional forms will be used.
Important: Save this link or the file in a secure location, as it will be used for all events throughout the year. Access to this form is restricted to your school and SOO staff only.
For each school, we maintain a database of previously registered students. Your forms include a list of all students previously registered from your school.
Actions Required:
• Remove: Identify and remove any students who are not participating this year (e.g., those who have graduated or left the school).
• Update: Complete the missing information for the remaining students to ensure their records are current.
Form Details: The first tab labeled “School Year” contains the intake form, which requires comprehensive information about your school. Ensure that all fields are completed and review the participating schools’ policy and code of conduct before signing. SOO will approve each form with a checkmark at the bottom. If any issues arise, SOO staff will leave a comment on the sheet.
• Important Notes:
1. Single Entry: This information needs to be entered only once.
2. Updates: You can update your list of participating students (both SO athletes and Unified Partners) throughout the year. Please highlight any changes.
Schools will receive an invoice in January based on the number of students registered.
Registration Process: After completing the intake form, you’ll need to fill out the respective form for each event you wish to participate in. Submit these forms at least 7 days before the event to ensure your registration is accepted.
Steps to Follow:
- Review Key Notes: Check for any important updates or changes.
- Fill in Required Information: Complete all necessary fields on the right-hand side of the form.
1. Application (only need to complete once)
Schools interested in becoming a Unified Champion School must complete this application once, including returning schools. This application is included in the registration file under the “Unified Champion School” tab. Please follow these steps:
- Answer the Short Questions: Provide responses to the initial questions at the top of the form.
- Select Pillar Initiatives: Indicate which pillar initiatives you aim to achieve. (Note: This is a projection of your goals.)
- Outline Your Plan: Briefly describe how you plan to achieve these initiatives.
- SOO will mark your application with a checkmark to confirm receipt. If there are any issues, SOO staff will leave a comment on the sheet.
2. Annual Report – Due April 30th
To maintain your status as a Unified Champion School or Banner School, you must submit an annual report by April 30th each year. This report should include:
- Participation Numbers: Detail how many people participated.
- Initiatives Achieved: Specify which initiatives your school accomplished.
- Successes and Challenges: Provide a detailed account of successes and challenges faced, using the required long-answer questions.
