  1. Legal First & Last Name – as it appears on Birth Certificate or Passport
  2. Date of Birth
  3. Mailing Address
  4. Emergency Contacts – you should provide at least one contact name and phone number of who can be contacted in case of emergency.

Parent/Guardian Info

  1. Legal First & Last Name – as it appears on Passport or Police Check
  2. Valid Email Address
  3. Date of Birth
