Report 58 is almost perfect for our needs for what we call an “athlete info sheet” that we give to all head coaches/team managers for use in case of emergency! Unfortunately, until Doctor’s name and phone number (and ideally individual’s address) is included, I will still need to do a mail merge using info from a merge of the excel spreadsheet reports to continue to produce the info sheets.

Please add: Doctor’s Name, Doctor’s Phone number, individual home address. Sex and gender identify would also be nice (to avoid confusion for transgender members if nothing else) but is not absolutely necessary.

Thanks for the feedback. We’ll make a note of this, all good points. I know they are balancing a fine line between things used all the time and things referenced, but good suggestions. We’ll add it to the list.
