Recognised today that Report 54 – Individual Athlete Report does not print the athletes home address. Suggestion (print beside the 3 lines used for email addresses). Also, within the medical information the Doctor’s Name and contact information is not included. We would require that information if we needed to quickly get an athlete to the hospital in an emergency situation. Suggestion (add beside the health card number).
